2024 SEASON RESISTRATION

Week #1 – July 22 to 26

Junior Pro (8 and under) – 10 spots
Elite (9 – 11) – SOLD OUT – Space for 1 goalie
Senior (12 and up) – SOLD OUT – Space for 3 goalies

Week #2 – July 29 to August 2

Junior Pro – 14 spots
Elite – SOLD OUT – Space for 1 goalie
Senior – SOLD OUT – Space for 3 goalies

WAIVER AGREEMENT (REQUIRED)

I hereby authorize the staff of Mike Fountain Muskoka Summer Hockey School to act for me according to their best judgement in any emergency requiring medical attention and I hereby waive and release the school and its staff from all liability for any injuries incurred at the said school. 

This waiver also excludes the participant from his/her rights to sue said hockey school for damages and injuries incurred at the school.Photogrpahy Waiver: It is agreed by submitting this registration form that photographs may be taken of the participant in the Mike Fountain Muskoka Summer Hockey School which may be used for promotional purposes, including the official website for this hockey program. If you do NOT agree please contact the school at (705) 687-1688
